Background: HIV case numbers remain high globally, nationally, and specifically in Bitung City, which ranks second in the cumulative number of HIV/AIDS cases in North Sulawesi Province. Despite various prevention efforts, the rate of new case detection remains concerning, necessitating more effective prevention innovations.

Purpose: To assess the readiness of Community Health Centers (Puskesmas) in Bitung City to provide Pre-Exposure Prophylaxis (PrEP) services as an HIV prevention strategy.

Method: This study employed a qualitative approach. Informants included various stakeholders involved in the HIV program: HIV program managers at the Health Office, heads of health centers, physicians, nurses, HIV counselors, laboratory staff, pharmacy personnel, HIV/AIDS information system managers, NGO representatives, community leaders, and representatives from key populations who are potential PrEP users. Data were collected through in-depth interviews, observations, and document reviews.

Results: Overall, all informants assessed that the health centers in Bitung City possessed the basic readiness to develop PrEP services. Basic facilities and infrastructure—such as consultation rooms, HIV testing laboratories, and other service facilities—were available, although some health centers still faced limitations regarding private consultation spaces, laboratory facilities, and general infrastructure.

Conclusion: Health centers in Bitung City possess the basic readiness to provide PrEP services, particularly regarding the availability of human resources, infrastructure, HIV/AIDS information systems, and pharmaceutical management systems that already support HIV services. However, this readiness has not yet reached an optimal level; further efforts are needed to strengthen healthcare worker capacity through specialized PrEP training, ensure the supply of medication and logistics, improve services to guarantee privacy, and enhance education to reduce stigma against HIV and key populations.

Suggestion: Future research should evaluate the effectiveness of PrEP service implementation once the program is underway and develop community-based intervention models to enhance the success of HIV prevention in the future.
