Background: Hypertension is a health problem that can lead to serious complications if not properly controlled. In addition to pharmacological therapy, dietary management in accordance with recommended guidelines is an important component of hypertension control. Knowledge of hypertension and dietary management may be associated with patients’ adherence to recommended dietary practices.

Purpose: To determine the relationship between the level of knowledge and dietary adherence among patients with hypertension at Pringsewu Primary Health Center in 2026.

Method: This study employed a quantitative approach with a cross-sectional design. It was conducted from April to May 2026 among 124 patients with hypertension residing in East Pringsewu Village who received treatment at Pringsewu Primary Health Center. Participants were recruited using consecutive sampling. Data were collected using a hypertension knowledge questionnaire and a hypertension dietary adherence questionnaire. The association between the variables was analyzed using the Pearson Chi-square test.

Results: Most respondents had a high level of knowledge, comprising 65 respondents (52.4%), while the largest proportion demonstrated moderate dietary adherence, comprising 44 respondents (35.5%). Cross-tabulation showed that 28 of the 65 respondents with a high level of knowledge had high dietary adherence. The Pearson Chi-square test indicated a statistically significant relationship between the level of knowledge and dietary adherence (χ² = 18.852; p < 0.001).

Conclusion: There was a statistically significant relationship between the level of knowledge and dietary adherence among patients with hypertension at Pringsewu Primary Health Center. Health education regarding hypertension and dietary management may be considered one of the strategies to support patients’ dietary adherence.
